To Pass Collateral Clearance Information

For DOE personnel or individuals with a DOE clearance, complete the DOE F 5631.20.

For all other agencies, have your security staff provide the following information in memo form:

• First and last name
• Social Security Number
• Date of birth
• Clearance to be passed
• Date(s) of visit
• Purpose of visit
• Argonne point of contact
• Parent organization or agency
• Make and model of any worn medical devices (such as hearing aids, heart pacemakers, or glucose monitors)

Send the form or memo with password protection or encrypted email to AIC@​anl.​gov.

To Pass SCI Access

Information can be passed to Argonne National Laboratory through the following means:

• In memo format via unclassified fax at (630) 252-5678 (receiving machine is in a locked location)
• In memo format with password protection or encrypted email to SSO@​anl.​gov
• Via M3 using PLA address ​“DOE SSO//ANL//”

Note that Argonne does not have JPAS and cannot accept SMO codes.

Include the following information:

• First and last name
• Social Security Number

• Date(s) of visit
• Purpose of visit
• Argonne point of contact
• Parent organization or agency
• Parent SSO POC (include email and phone number)
